ConnectiCare members (and their caregivers) can get personalized help from nurse care managers, a certified diabetes educator, or a social worker at our centers in Manchester and Waterbury. They can help with:
Learning to use a glucometer
Healthy eating for individuals with diabetes
Finding senior meal programs, elder care, social activities, and support groups
Guiding members to community resources
Managing a chronic condition
Learning about your prescription medicines
Learning to use a blood pressure cuff
There’s no charge to members. It’s all part of what we do to try to keep our members healthy.
